PROGRAMS OFFERED:

Community Care Programs (CCP):

The Community Care Program aims to assist older adults in maintaining their independence and providing cost-effective alternatives to nursing home placement. These services include IN-HOME SERVICES, RESPITE CARE, and 24HR LIVE-IN CARE.

ARE YOU ELIGIBLE?

✔60 years old or older.
✔Either U.S. citizens or eligible non-citizens within the specific categories.
✔Residents of Illinois.
✔Have non-exempt assets of $17,500 or less (Your home, car, or personal furnishings are classified as exempt assets.); and
✔Have an assessed need for long-term care (to be at risk for nursing facility placement as measured by the Determination of Need (DON) assessment).
✔Apply for and, if eligible, enroll in Medicaid

IN HOME SERVICES

This Community Care Program service provides assistance with household tasks such as cleaning, planning and preparing meals, doing laundry, shopping and running errands. Home Care Aides also assist clients with personal care tasks such as dressing, bathing, grooming and following special diets.

RESPITE CARE

This short-term service provides temporary relief for the assigned homemaker or family members responsible for the care of our client. Relief staff will be scheduled for the authorized days and hours requested by the assigned staff.

VETERANS AFFAIRS

This program is for Veterans who need personal care services and help with activities of daily living. ZAYAN LLC works with the US department of veteran affairs to provide trained and compassionate homemakers/ Home health Aides as a part of an alternative to nursing home care. The Homemaker/Home Health Aide services can be used in combination with other Home and Community Based Services.

DIVISION OF REHABILITATION
